不同血液净化方式在慢性肾衰竭患者血液透析治疗中的效果对比(1)

     [摘要]目的 探討不同血液净化方式对血液透析(HD)患者血清β2微球蛋白的清除效果。方法 选取2016年3月~2018年3月在我院接受长期维持型血液透析治疗的100例慢性肾衰竭患者作为研究对象,随机分为低通量透析(LFHD,n=21)、高通量透析(HFHD,n=23)、血液透析及血液透析滤过(HD+HDF,n=28)和血液透析联合灌流(HD+HP,n=28)四组,分别采用LFHD、HFHD、HD+HDF、HD+HP的方式进行血液净化治疗。比较四组血清β2微球蛋白清除效果,并比较四组并发症的改善情况。结果 HFHD、HD+HDF及HD+HP组治疗后的β2微球蛋白明显低于治疗前,差异有统计学意义(P<0.05)。治疗后HD+HDF及HD+HP组的β2微球蛋白明显低于其他两组,差异均有统计学意义(P<0.05);四组经不同的HD治疗后皮肤瘙痒、骨痛、行走困难并发症均有不同程度的改善,HFHD、HD+HDF、HD+HP三组在并发症改善的总有效率明显高于LFHD组,差异有统计学意义(P<0.05)。结论 采用HD、HP联合HD的方式血液净化治疗,能更有效清除慢性肾衰竭患者的血清β2微球蛋白,对其相关并发症的改善效果更好,值得临床推广应用。

    Comparison of the effects of different blood purification methods in the treatment of dialysis in patients with chronic renal failure

    [Abstract] Objective To explore the clearance effect of different blood purification methods on serum β2 microglobulin in hemodialysis (HD) patients. Methods From March 2016 to March 2018, 100 patients with chronic renal failure who received long-term maintenance hemodialysis treatment in our hospital were selected as subjects and randomly divided into four groups: low flux dialysis group (LFHD, n=21), high throughput dialysis group (HFHD, n=23), hemodialysis and hemodialysis filtration group(HD+HDF, n=28) and hemodialysis combined perfusion group(HD+HP, n=28). They were treated with LFHD, HFHD, HD+HDF, HD+HP respectively. The clearance effect of serum β2 microglobulin were compared among the four groups, and the improvement of complications in the four groups were compared. Results After treatment, the β2 microglobulin in the HFHD, HD+HDF and HD+HP groups were significantly lower than those before treatment, the differences were statistically significant (P<0.05). After treatment, the β2 microglobulin in the HD+HDF and HD+HP group were significantly lower than that in other two groups (P<0.05). After different hemodialysis treatment, the complications of skin itching, bone pain and walking difficulty were improved in all four groups, the total effective rate of HFHD, HD+HDF, HD+HP group were significantly higher than those of LFHD group, the differences were statistically significant (P<0.05). Conclusion Blood purification with HDF, HP combined with HD can effectively remove serum β2 microglobulin from patients with chronic renal failure and improve its related complications, which is worthy of further clinical application., 百拇医药(欧玉玲 张培光)
