[摘要] 目的:探讨2型糖尿病患者血脂异常与大血管病变之间的关系。方法:对111例2型糖尿病患者,依据颈动脉及下肢动脉血管彩色超声多普勒检查分为无血管病变组、轻度血管病变组和重度血管病变组,收集其病史、临床及实验室检查资料进行统计及回顾性分析。结果:重度血管病变组血清低密度脂蛋白(LDL)、载脂蛋白B(ApoB)水平明显高于无血管病变组、轻度血管病变组,差异有统计学意义(P<0.05),轻度血管病变组LDL明显高于无血管病变组,差异有统计学意义(P<0.05),无血管病变组与重度血管病变组血清脂蛋白(a)[Lp(a)]相比,差异有统计学意义(P<0.05),多元Logistic回归分析显示年龄、高血压病程、LDL、ApoB是2型糖尿病大血管病变的重要危险因素。结论:2型糖尿病大血管病变与血清LDL、ApoB、Lp(a)有明显相关性;其中年龄、高血压病程、LDL、ApoB是大血管病变的重要危险因素。

Relationship between lipid metabolism abnormality and macrovascular disease in type 2 diabetes

[Abstract] Objective: To investigate the relationship between lipid metabolism abnormality and macrovascular disease in type 2 diabetes. Methods: 111 patients were divided into nonvasculopathy group,mild vasculopathy group and severe vasculopathy group according to the results of carotid ultrasonography. History,physical examination, laboratory examination were detected in each group and give retrospective analysis. Results: The level of low density lipoprotein(LDL-C) and apoprotein B(ApoB) in severe vasculopathy group was significant higher than that of mild vasculopathy group and nonvasculopathy group(P<0.05). The level of low density lipoprotein(LDL-C) in mild vasculopathy group was significant higher than that of nonvasculopathy group(P<0.05).There was significant difference between nonvasculopathy group and severe vasculopathy group in the level of serum lipoprotein(a)[Lp(a)](P<0.05). Result of multiple regression analysis revealed that age,course of hypertension,LDL-C and ApoB were the significant risk factors of macrovascular disease. Conclusion: Macrovascular disease is correlated with LDL-C,apoB and Lp(a) in type 2 diabetes,The significant risk factors are age, course of hypertension,LDL-C and ApoB.
